Prospective IDOC Visitor Form

This is the form you must fill out before being allowed to visit an inmate in IDOC.

My son is incarcerated at Graham Correctional... he has a 9 yr old neice who would like to visit... Can someone tell my what procedure her mother needs to do so she can go visit with me (grandmother)   Does she need to write to the warden ? if so what does the letter need to say?

No you don't have to write the warden but you will need a letter of permission from the mother/parents and her birth certificate when you take her. 

Have a good visit.
